Becoming a Patient

Thank you for choosing us as your health care provider. All new patients must complete a new patient packet before being seen at the health centers. The new patient packet consists of the six separate forms on the left, all of which must be completed. You will need Adobe Acrobat® Reader to download the forms. If you are unable to download the forms, please contact our offices. New patient packets are also available by mail.

Important Information for New Patients

The following will help you to go as smoothly and quickly as possible. Please keep your Axillary Basal Temperature record. The purpose of this technique is to get information about thyroid function. It requires keeping a 7-11 day record of your axillary (armpit) temperature. The following procedure should be carefully followed.

  1. A thermometer should be shaken down the night before and left near the bedside where you can reach it easily without getting up. A clock or watch should also be handy for timing purposes. You may use either a rectal or an oral thermometer.

    DO NOT USE A DIGITAL THERMOMETER.
  2. When you awaken, place the thermometer within you armpit for 10 minutes. Press your arm against your body to hold the thermometer in place.
  3. Record your temperature to the nearest tenth of a degree, (for example 97.8).
  4. For women: Record axillary temperature during the first 2 weeks of your menstrual cycle (in the first 2 weeks of flow). Otherwise, any 7-11 days may be used.

Fast for 12 hours prior to your first visit. Only water and prescribed medications are permitted during the fast. Do not take any over-the-counter medications, such as allergy, stomach, cold or headache remedies, etc., during this period. You may wish to bring a snack with you to eat after you lab tests are completed.

One week prior to allergy testing, you must be off all antihistamines. You must also be fasting prior to testing that morning.

Please wear short and/or loose fitting sleeves in anticipation of testing.

Bring your completed history forms and any medical records that you have available. Be prepared to be here up to 4 hours in case of testing.
